Patient Acceptance Stages

1- Birth Certificate (in case of no birth certificate)

2- Copy of birth certificate (copy of birth certificate and parent card if no birth certificate and birth certificate)

3- Copy of the baby’s first copy of the book 2 copies (copy of the mother’s book if there is no insurance book)
4- A day of hospitalization with a first-degree companion (parents or siblings over 18 years of age) is required for hospital admission if the infant or child requires surgery if the patient’s father is otherwise eligible. Patient with legal guardian

1- Copy of angiographic response, echocardiographic report

2- Copy of the test results (HIV, HCV, HBS) and blood X-rays

3- Spirometry and Doppler ultrasound, if necessary

4- If you have anticoagulants such as Plavix, Aspirin, Oswex, consult your doctor before being admitted.

5- Tell your doctor if you have a cold, fever, sore throat, respiratory, administrative, or gastrointestinal infection a few days before hospitalization.

6- Pre-operative dental examinations are required if cardiac valve surgery is required.
7- Includes a copy of the patient’s birth certificate and national ID card
8- Carrying a copy of the Patient Insurance Booklet (2 copies)
9-On the day of hospitalization, it is mandatory to have a first-degree companion (father, wife, child ..)

5- Third party (driver) insurance policy All documents must have a copy equal to the original of the checkpoint
6- A letter from the Secretariat to the post office is required to complete the above documents

6- It is recommended that all gynecological surgical treatment be accompanied by the patient on the day of hospitalization

5- The presence of the patient’s spouse is essential for satisfaction on the day of hospitalization.

For patients who need to be replaced jointly and are covered by Social Security insurance, a copy of the original insured’s birth certificate is required.
Patients requiring arthritis replacement and covered by health insurance must have a 5-year insurance payment history (if not included in the insurance cover, you should receive mail from the Post Office and get the approval of a resident expert)
